FUNCTIONAL CAPACITY AND LIVING SKILL ASSESSMENT
Our occupational therapists offer high quality assessment, delivered online. We choose from a range of assessment tools to tailor the assessment to your needs. These may include questionnaires that can be based on therapist observation, or your self-report, or both. These are combined with a structured interview where there the OT discusses in detail, how your mental health condition or disability impacts on your daily life, including the things you want, need and are expected to do. You are welcome to have a support person accompany you during the assessment. Sometimes, your OT will seek your permission to speak to other people, such as carers or family, or services that know you well. This allows your OT to gather further information. Your OT may ask you to send videos of how you complete certain tasks, to gain greater understanding of the challenges you face daily.
THERAPEUTIC SUPPORT
Our experienced mental health occupational therapists offer a range of therapeutic approaches to enable you to reach your goals. Tell us what you want and need to do, and we will tailor an approach that will build your capacity to work towards your priorities. We offer daily living skill intervention, goal attainment scaling, sensory-based interventions, and motivational approaches, and others. Our approach is trauma informed and led by you, the client. We will offer you a detailed therapist bio early on, to help you choose and get to know your OT.
BOOKING AND FEES
We aim to make booking as simple as possible for you, the client. You can book by calling or emailing us directly, and we will respond to discuss a suitable time. We can answer any questions you may have. We will also share fee details on first contact. We are NDIS-registered and Medicare providers.
We will share some tips on how to reduce the time needed to complete a functional assessment, thereby reducing the cost also, for example, by gathering relevant information prior to the assessment.
Payment is requested following assessment, and prior to release of the occupational therapy report.
FREQUENTLY ASKED QUESTIONS
Why might I need a functional capacity or living skill assessment?
Your assessment report is a tool which accurately describes your abilities, and difficulties you experience in your daily living as a result of illness or disability. It will also describe the supports and strategies that would be most beneficial to you in overcoming functional and life-challenges. You may choose to use your report to communicate your needs, for example as you apply for funding, or choose providers who can support you to overcome daily challenges. Your report can be particularly helpful to describe your functional capacity as part of an NDIS application or appeals process.
What technology do I need?
You, or your support person, will need access to a computer or laptop with a camera, access to the internet, and an email address. We will send you a link contained in an email address, and by clicking on this at the agreed time, you will meet your therapist and commence the assessment. You can choose to have a support person help and guide you through the assessment planning stages and completion of the assessment.
